What is a Transition Plan and What is it For?

Transition plans are documents containing a wide range of information about an Individual who will be experiencing significant change in their life. Change sufficient to warrant a Transition Plan include such things as; progress from high school to a day program or agency, from independent living to an extended care environment, from unemployment to employment, or even just between grades or schools. Transition plans can be used to make sure all those involved with the Individual experiencing change are best able to help them in their adjustment.

Transition plans contain a wide range of information including:

• historical information
• recreation preferences
• medical issues
• needs and strengths
• guidelines for information gathering and organization

Who creates the Transition Plan?

A Transition Plan is created by all those who are familiar with the abilities, strengths, needs and requirements of the Individual. This can include parents, teachers, children, psychologists, doctors, councilors and the Individual themself.

What do you do with your Transition Plan once it’s completed?

Once accurate and relevant information is compiled and ready, it can be made available to those who don’t know the individual as well, and who will be working with him or her. A concise record of medications, behavioral issues and effective (and ineffective) ways of dealing with problems can be invaluable to workers.

Once you have your Transition Plan completed you can give a copy to the previous teacher, new teacher and school councilor if required if the Individual is in the school system.

If the Individual is progressing from a school environment to an agency day program, the agency would receive a copy of the Transition Plan.

Other situations which could warrant the use of a transition plan also include: moving between schools, transitions between hospitals and home or school, and between other environments. There are many more.
